Job summary

Apex Home Health, Hospice & Home Care seeks a Travel Physical Therapist to evaluate, diagnose, and treat patients with mobility impairments, reduce pain, and improve quality of life in diverse settings in Hawaii.

You will develop personalized, evidence-based treatment plans, using manual therapy and exercise, to facilitate recovery and independence while coordinating with healthcare professionals and educating patients and caregivers.

Qualifications

• Valid physical therapy license per state regulations.
• BLS certification (AHA/ARC) current.
• Strong clinical, communication, and critical thinking skills.
• Proficiency in PC and ability to follow instructions.
• Physical ability to move and lift equipment (50–100 lbs).
• Effective interpersonal skills and professionalism.
• Adaptability to irregular schedules and ability to work under pressure.
• Good visual acuity and communication abilities.

Responsibilities

• Assess patients' physical conditions through tests, examinations, and history-taking.
• Create and implement individualized rehabilitation programs.
• Operate and maintain specialized physical therapy equipment.
• Monitor progress, adjust treatment plans, and document outcomes.
• Collaborate with healthcare professionals for coordinated care.
• Educate patients and caregivers on exercises, injury prevention and self-management.
• Maintain a safe, compliant treatment environment and participate in QA.
• Assist in discharge planning and support continued patient recovery.
