Job summary

Plan A Health is seeking a Physical Therapist (PT) to assess patients needing rehabilitation, develop and implement treatment plans, and provide skilled PT services per physician orders.

The role requires a Doctorate or Bachelor's degree in Physical Therapy with NPTE certification, completion of an accredited program, and current CPR certification. Licensure is required or in progress, with opportunities to collaborate across a multidisciplinary team.

• Must hold or be in process of obtaining state licensure.
• Current CPR certification is required.

Job description

Role: The Physical Therapist (PT) is responsible for assessing patients requiring rehabilitation, establishing and implementing treatment plans, and providing skilled physical therapy services in accordance with physician orders.

Key Duties: Develops individualized care plans, evaluates patient needs, modifies interventions as needed, documents patient progress, and educates patients and families on therapy methods. Collaborates with multidisciplinary teams to achieve treatment goals.

Qualifications & Skills: Must hold a Doctorate or Bachelor's degree in Physical Therapy with NPTE certification, have graduated from an accredited institution, possess a minimum of one year's experience, and hold or be in process of obtaining state licensure. Current CPR certification is required.
