• Implement evidence-based therapeutic interventions, document progress, and adjust plans based on patient response and goals.
• Collaborate with an interdisciplinary team (nursing, OT, SLP, social work) to optimize care plans, discharge planning, and family education.
• Administer therapeutic modalities as appropriate, monitor safety, and ensure infection control and patient protection during therapy sessions.
• Educate residents and families on home exercise programs, safety strategies, and fall-prevention techniques; provide clear patient- and caregiver-facing instructions.
• Maintain accurate documentation in the medical record, meet regulatory requirements, and contribute to quality improvement and compliant care delivery.
