Position Summary

Evaluate, design, and implement patient care plans; participate in community service. Develop plans to improve or restore function, prevent disability, and help patients reach their performance goals.

Responsibilities

• Manage the full patient cycle: evaluation, treatment planning, progress monitoring, physician communication, discharge, and follow-up.
• Accompany patients during physician visits.
• Mentor new staff.
• Complete timely documentation including evaluations, notes, reports, and insurance forms.
• Engage in community outreach activities such as hosting booths, presenting, providing screenings, etc.
• Build relationships with referral sources like physicians, PAs, ARNPs, chiropractors, and case managers.

Qualifications

• Degree from an accredited Physical Therapy Program.
• 1-2 years of relevant experience.
• Active clinician license or compact privileges in the state of service.
• Current CPR certification.

Language Skills

• Proficient in reading, writing, and speaking English.

Compensation

Minimum: USD $63,000/year; Maximum: USD $100,000/year.
