Job summary

Healthforce Healthcare LLC is seeking an Occupational Therapist to provide 13-week travel therapy in a skilled nursing facility in Pittsfield, MA. Start date 11/30/2026 with a guaranteed 35 hours per week and a daily shift from 08:00 to 16:30.

The role requires licensure or eligibility in Massachusetts, NBCOT, and prior SNF experience. You will evaluate, plan, and implement OT interventions, document progress, and collaborate with an interdisciplinary team.

Qualifications

• Active Occupational Therapist license in Massachusetts or eligibility for licensure.
• NBCOT certification or current eligibility.
• Experience in a skilled nursing facility or long-term care setting preferred.
• Strong documentation and communication skills; ability to collaborate with an interdisciplinary team.
• Ability to work a 13-week travel assignment and comply with the 08:00–16:30 schedule.

Responsibilities

• Conduct comprehensive OT evaluations, establish goals, and design individualized treatment plans for SNF residents.
• Implement evidence-based interventions to improve ADLs, mobility, cognition, and upper-extremity function; provide adaptive equipment recommendations as appropriate.
• Document assessments, treatment plans, progress notes, and discharge planning per department standards and regulatory requirements.
• Collaborate with the interdisciplinary team, including families and caregivers, to support resident goals and participate in care planning meetings.
• Reassess resident progress regularly; adjust goals and interventions; support safe transfers and discharge preparation.
• Maintain compliance with infection control, safety protocols, and facility policies during therapy sessions.
