Responsibilities

• Assess residents, establish measurable goals, implement treatment sessions, and document progress in the charting system.
• Develop and adjust treatment plans based on resident response, incorporating therapeutic activities to promote independence and safety.
• Educate residents and caregivers on adaptive equipment, transfer techniques, activity modification, and fall-prevention strategies.
• Collaborate with an interdisciplinary team (OT, PT, SLP, nursing, social work) to coordinate care and support discharge planning.
• Maintain infection control practices, ensure safety during therapy sessions, and comply with regulatory standards and documentation requirements.
