Responsibilities include resident assessments, individualized treatment planning, interdisciplinary collaboration, and discharge planning. Credentialing and ongoing support are provided to travel clinicians.

Qualifications

• Active Maryland OT license or eligibility to obtain licensure.
• NBCOT certification or eligibility.
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
• Ability to work in a SNF environment and participate in a travel assignment.
• Strong communication and documentation skills; proficient in common EMR systems.

• Assess residents to determine OT needs and develop individualized treatment plans aligned with goals.
• Implement and monitor therapeutic interventions to improve self-care, mobility, cognition, and independence.
• Administer standardized assessments, document progress, and adjust plans as needed.
• Collaborate with an interdisciplinary team to coordinate care and discharge planning.
• Educate patients and families on home programs, adaptive equipment, safety, and activity modification.
• Maintain thorough documentation in the EMR, ensuring regulatory compliance.

Requirements

• Active Maryland OT license or eligibility to obtain licensure
• Degree in Occupational Therapy from an ACOTE-accredited program (Master’s required; Doctorate acceptable)
• NBCOT certification or eligibility for certification
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
• Ability to work in a SNF environment and participate in a travel assignment
• Strong communication and documentation skills; proficient in common electronic medical record systems
