Job summary

Transistional Care Physicians of America is seeking a dedicated Nurse Practitioner to deliver primary and transitional care across Skilled Nursing Facilities, Assisted Living Facilities, and patient homes in Georgia. The NP will assess, diagnose, and manage a medically complex geriatric population with a focus on reducing hospitalizations and improving quality of life.

Qualifications

• Active and unrestricted Georgia APRN license.
• Board eligible or board certified NP.
• Experience in SNF/ALF/ILF, geriatrics, hospice, or palliative care preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive assessments in SNF/ALF/ILF settings.
• Diagnose and manage acute and chronic conditions.
• Develop and implement care plans.
• Prescribe medications per GA guidelines.
• Coordinate care with facility staff and families.
• Review labs/imaging and follow up promptly.
• Document encounters within 48 hours.
• Participate in interdisciplinary rounds and on-call shifts.
