Job summary

UnitedHealth Group is seeking a Nurse Practitioner in Georgia to join the SNF Patient Connect program. You will perform a comprehensive health assessment with the care team to support better care coordination and health outcomes in short stay and transitional settings.

The role emphasizes autonomy, collaboration with primary care physicians, nursing staff, and patients’ families, with a focus on documentation, coding, and gap closure to reduce hospital readmissions and improve care transitions.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations.
• Active and unrestricted driver’s license with access to reliable transportation.
