Paycom - ATS is seeking a full-time Nurse Practitioner in Lufkin, TX, with routine travel to affiliated Texas facilities. The NP will provide adult and geriatric care, collaborate with a multidisciplinary team, and manage chronic conditions with a focus on post-acute care and value-based care delivery.

The role requires strong assessment, care coordination, and documentation within state regulations and collaborative practice agreements.

Qualificações

• 2 years of NP experience in adult/geriatric or family practice preferred.
• Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease.
• Skilled in identifying risk factors, unmet needs, and gaps in care for seniors.
• Proficient in medication reconciliation and managing clinically complex patients.
• Graduate of an accredited NP program; active RN/NP licenses in state; board certified.

Responsabilidades

• Perform comprehensive health risk assessments for assigned Members.
• Collaborate with physicians, case managers, social workers, caregivers, and facility staff to coordinate care.
• Identify quality improvement opportunities and develop individualized care plans.
• Review and interpret medical, surgical, and social histories.
• Prescribe medications, order tests, and provide wellness counseling.
• Refer Members to PCPs or disease management programs as needed.
