Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across a defined coverage area. Travel up to 100% may be required, with potential overnights and flights depending on geography.

You will review medications, conduct physicals, and perform tests as ordered by health plans. We value board-certified NP/PA clinicians who can obtain multi-state licenses and travel to counties within states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Able to obtain/maintain multi-state licenses as needed.
• Able to cover counties within state and flex to other areas of business need.

Responsibilities

• Provide in-home health evaluations across assigned coverage area.
• Travel locally up to 100% and arrange overnight stays as needed.
• Conduct reviews of medication history and physical evaluations; perform additional tests per health plan orders.
• Conduct virtual visits when required and collaborate with Signify Health network.
