Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations nationwide. The role involves travel up to 100% of the time, conducting 45‑minute visits, reviewing medical history, performing physicals, and adding diagnostic tests when needed.

You’ll use an iPad and a clinical workflow without prescribing or altering current treatments. Eligible candidates hold active, unrestricted licenses, board certification, and multi-state eligibility; travel

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area.
• Able to cover all counties within state and flex to other areas of business need.

Responsibilities

• Travel locally and within licensed states to conduct in-home visits.
• Perform health evaluations including medical history and physical exam.
• Provide diagnostic and preventive services as indicated by health plan.
• Utilize an iPad and clinical workflow during assessments.
• Coordinate care and connect members to appropriate services.
