Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members across Ohio. This field-based role requires up to 85% travel, with flexible scheduling between 8am-8pm across a 7-day week. Base pay plus incentives, including sign-on bonuses and student loan repayment options.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state
• Valid driver's license and transportation for home visits
• Ability to navigate varied home environments during assessments

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings with PCPs to address care gaps
• Identify urgent/emergent situations and intervene as needed
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
