Job summary

Optum Home & Community Care in Colorado is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform annual in-home health assessments for Medicare Advantage and other plan members.

This field-based role requires travel across the entire state (about 85%), with travel expenses covered. The position offers flexible scheduling (8am-8pm across a 7-day week), no on-call, no weekends, and no holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted license in Colorado without disciplinary actions.
• 1+ years of clinical experience (family, geriatric, or home health preferred).

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Document findings and coordinate with primary care providers to address gaps in care.
• Educate members on disease processes, medications, and self-management.
• Identify urgent or emergent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
