Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) for field-based in-home assessments across four states. Flexible scheduling and travel are core to the role, with base pay plus incentives and a comprehensive benefits package.

This position emphasizes patient education, care coordination with PCPs, and addressing social determinants of health, delivering high-quality care in patients' homes within the Optum HouseCalls program.

Qualifications

• 1+ years of clinical experience (family, geriatric, or home health preferred).
• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted license in the state you practice in, without disciplinary actions.
• Active and unrestricted driver's license; ability to work in varied home environments.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Educate members on disease processes, medications and compliance.
• Document diagnoses for care management and coordinate with PCPs to close gaps in care.
• Identify urgent situations and intervene appropriately; refer to needed services.
• Address social determinants of health and provide referrals as needed.
