Molina Healthcare Inc. seeks a Nurse Practitioner to provide screening, preventive and primary medical care across in-home, community, nursing facilities and pop-up clinics. You will coordinate care, document plans, and refer to specialists as needed while traveling within the assigned region.

The role requires NP certification, active state license, DEA, BLS, and strong communication skills with the ability to work independently and manage multiple priorities in diverse settings.

Qualifications

• At least 1 year of NP experience or equivalent.
• Active national certification (AANP or ANCC).
• Current FNP license in state of practice.
• DEA license with authority to prescribe.
• BLS certification.
• Valid driver's license and reliable transportation.

Responsabilités

• Provide medical care and care coordination for diverse member populations.
• Perform comprehensive evaluations and preventive assessments.
• Address chronic and acute complaints and determine urgency.
• Create and implement a medical plan of care.
• Schedule visits and provide post-discharge coordination.
• Conduct in-home and community-based visits with possible travel.
