Job summary

Molina Healthcare seeks a Nurse Practitioner to deliver screening, preventive and primary care across in-home, community, and facility settings. You will manage care plans, order tests, and coordinate with physicians to ensure outcomes while traveling regionally.

Your role involves performing exams, documenting in EMR, and providing post-discharge coordination. A valid FNP license, DEA, BLS, and strong communication are required, with up to 120 miles daily travel within the region.

Qualifications

• At least 1 year of experience as a nurse practitioner or equivalent
• Active and unrestricted national certification (AANP/ANCC)
• Current state-issued Family Nurse Practitioner license (FNP)
• Active DEA license to prescribe
• Current BLS certification
• Valid driver's license and reliable transportation
• Ability to work in diverse settings and with varied populations
• Independent work style with minimal supervision
• Strong communication skills and responsiveness
• Proficiency with EMR and Office software

Responsibilities

• Provide screening, preventive and primary care in home and community settings
• Perform comprehensive history and physical exams to identify gaps in care
• Order and interpret diagnostic tests and coordinate with physicians
• Develop and implement individualized plans of care
• Schedule follow-up visits and coordinate post-discharge care
• Travel within assigned region (up to 120 miles daily) and possibly overnight stays
• Collaborate with physicians to develop best practices
• Document medical records timely in EMR
• May prescribe medications and perform procedures as allowed by state law
