Job summary

Molina Healthcare is seeking a Nurse Practitioner for home health and community-based patient care. You will provide primary care, coordinate care plans, and perform periodic assessments across diverse settings.

The role requires travel within the region, independent work, and collaboration with physicians to ensure quality outcomes. Applicants should have NP certification (AANP/ANCC), active FNP license, and a current DEA license.

Qualifications

• At least 1 year of experience as a nurse practitioner.
• Active national NP certification (AANP or ANCC).
• Current unrestricted FNP license in practice state.
• Active DEA license to write prescriptions.
• BLS certification is current and valid.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.

Responsibilities

• Provide general medical care and care coordination across patient populations.
• Perform history, physical exams, and preventive assessments.
• Order and interpret diagnostic tests as appropriate.
• Develop and implement care plans and engage in post-discharge coordination.
• Conduct face-to-face visits in homes, community sites, and clinics.
• Travel within assigned region, up to 120 miles daily.
