Optum Home & Community Care, part of the Optum family, is expanding our Senior Community Care team to provide integrated care in nursing homes, senior housing and assisted living settings. This role requires travel across PA, DE, NC, SC and GA, with expenses covered.

We offer a base salary plus incentives and a strong benefits package. As a Senior APC, you will conduct comprehensive assessments, support sub-acute rehabilitation, document thoroughly, and engage in advanced care planning with

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure (PA/NC/SC).
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with access to reliable transportation.
• Ability to gain a collaborative practice agreement if applicable.
• Willingness to travel across PA, DE, NC, SC and GA (approx. 85%).

Responsibilities

• Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs and physical exam.
• Support members during a short-term sub-acute rehabilitation episode.
• Ensure accurate and complete documentation, including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.
