Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plans across North Carolina. The role is field-based with 85% travel, covering the entire state, and offers a base salary plus incentives.

Travel expenses are covered, and scheduling is flexible within a 8am–8pm window across a 7-day work week.

Qualifications

• NPs: Active ANCC or AANP certification in Family/Adult/Geriatric Primary Care
• PAs: NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state without disciplinary actions
• Active and unrestricted driver’s license with reliable transportation for home visits
• Able to navigate varied home environments and perform assessments
• If practicing independently, obtain licensing board approval within required timelines

Responsibilities

• Conduct comprehensive in-home assessments including med history review, vitals, and exam
• Identify and document diagnoses for care management and planning
• Communicate findings to PCPs to address care gaps
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on diseases, medications, and compliance
• Address social determinants of health and provide referrals as needed
