Job summary

Optum Home & Community Care in North Carolina is seeking a Nurse Practitioner or Physician Assistant for HouseCalls. This field-based role conducts annual in-home health assessments for Medicare Advantage and other plans, with flexible scheduling from 8am–8pm across a 7-day week. No on-call, weekends, or holidays.

Compensation includes a base with potential bonuses, and a diverse benefits package. Active licensure in NC and related certifications are required for employment with Optum.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure for PAs.

Responsibilities

• Conduct in-home health assessments.
• Perform vital signs, medications reconciliation, and exams.
• Identify diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care.
• Educate patients on disease processes and medications.
• Address social determinants of health and provide referrals.
