Job summary

UnitedHealth Group in Winston-Salem, NC, is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with PCPs.

The position offers flexible scheduling between 20-40 hours per week, no on-call, no weekends, and no holidays. Requires active NP/PA certification and state licensure, plus travel to member homes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
• Active and unrestricted driver's license and access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, vital signs, and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Educate members on disease processes, medications, and adherence; address social determinants of health
