Job summary

Sweetwater Care seeks an experienced LVN supervisor to oversee nursing care in a long-term care setting. You will implement physician orders, educate families and patients, and coordinate with departments on patient changes.

The role requires timely medication and treatment administration and rounds with physicians as needed. Ideal candidates hold a California LVN license (or compact state license) and have prior nursing management experience, with familiarity in long-term care and

• Must be licensed LVN in California if contract is in CA; compact license accepted in compact states.
• Minimum 1 year nursing management experience preferred.
• Experience in long-term care settings.
• Experience with PointClickCare charting.

Responsibilities

• Provides nursing care within scope of practice per standards, orders, and policies.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Makes timely medication administration per facility policy.
• Delivers timely treatments per policy.
• Receives and carries out physician orders, when permitted by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred

• POINT CLICK CARE CHARTING EXPERIENCE
