Job summary
Healthforce Healthcare LLC is seeking a Licensed Practical Nurse for a travel assignment in Crivitz, Wisconsin. Start date is 10/26/2026 with a guaranteed 36 hours per week and a flexible shift in a long-term care/assisted living facility.
The position offers weekly pay and a duration expressed in weeks. You will work with an interdisciplinary team to deliver care and ensure resident safety in a travel assignment managed by Healthforce.
Qualifications
- Active LPN license in good standing.
- Current CPR/BLS certification.
- Experience in long-term care/assisted living preferred.
- Ability to adapt to flexible scheduling and work in LTC settings.
- Pass standard health screenings and background checks.
Responsibilities
- Deliver direct nursing care to residents in a long-term care/assisted living setting per care plans.
- Administer medications and treatments per physician orders; monitor for side effects and document responses.
- Conduct routine assessments, monitor vital signs, and report changes to the care team.
- Collaborate with interdisciplinary team to coordinate care and support activities of daily living.
- Maintain charts and documentation in the EHR; adhere to regulatory and infection control protocols.
- Respond to changes in condition, provide triage, and assist with emergency procedures as trained.
