Job summary

HealthForce Healthcare is seeking an LPN for a travel assignment in Crivitz, Wisconsin. This position involves direct resident care in a long-term care/assisted living setting, with a flexible shift schedule and start date of 10/26/2026.

Expect weekly pay and guaranteed 40 hours per week. Assignment length varies, with onboarding and credentialing support provided by HealthForce Healthcare LLC and 24/7 recruiter assistance.

Qualifications

• Strong communication and teamwork skills are required.

• Knowledge of safety standards and accurate documentation practices.

Responsibilities

• Administer prescribed medications and treatments according to physician orders and facility policy.
• Monitor residents’ health status, document observations, and promptly report changes.
• Provide direct bedside care and assist with activities of daily living (ADLs) as needed.
• Assist with wound care, routine nursing procedures, and infection control practices.
• Collaborate with the interdisciplinary care team to update plans and communicate changes.
• Maintain accurate documentation in the electronic health record and ensure resident privacy and safety.
