Qualifications

Responsibilities

• Deliver direct nursing care to residents in a long-term care/assisted living setting per care plans.
• Administer medications and treatments per physician orders; monitor for side effects and document responses.
• Conduct routine assessments, monitor vital signs, and report changes to the care team.
• Collaborate with interdisciplinary team to coordinate care and support activities of daily living.
• Maintain charts and documentation in the EHR; adhere to regulatory and infection control protocols.
• Respond to changes in condition, provide triage, and assist with emergency procedures as trained.
