Job summary

HealthForce Healthcare is seeking an experienced Travel Licensed Practical Nurse (LPN) for a Long-Term Care facility in Dubuque, IA. The role offers a day shift, full-time travel contract with weekly pay and the chance to deliver high-quality, patient-centered care in a fast-paced setting.

Responsibilities include medication administration, vital signs monitoring, wound care, and comprehensive documentation in the EHR.

Qualifications

• A Practical Nursing program graduate.
• Current, unencumbered LPN license in Iowa or active Compact/NLC state license.
• Active Basic Life Support (BLS) / CPR certification.
• Minimum 1+ year experience in long-term care or SNF.
• Strong verbal and written communication and proficiency with EHR documentation.

Responsibilities

• Administer medications, monitor vitals, perform wound care per care plans.
• Continuously assess resident health changes and report to RNs, physicians, and families.
• Collaborate with CNAs, therapists and multidisciplinary teams to support ADLs and rehab goals.
• Maintain accurate resident records in the EHR in compliance with safety standards.
