Qualifications

• A Practical Nursing program graduate.

• Minimum 1+ year experience in long-term care or SNF.
• Strong verbal and written communication and proficiency with EHR documentation.

Responsibilities

• Administer medications, monitor vitals, perform wound care per care plans.
• Continuously assess resident health changes and report to RNs, physicians, and families.
• Collaborate with CNAs, therapists and multidisciplinary teams to support ADLs and rehab goals.
• Maintain accurate resident records in the EHR in compliance with safety standards.

Key Responsibilities

• Resident Care & Monitoring: Administer medications, monitor vital signs, perform wound care, and execute routine nursing procedures following individualized care plans
• Assessment & Reporting: Continually assess changes in resident health conditions and promptly communicate updates to RNs, physicians, and resident families
• Collaboration & Support: Work alongside CNAs, therapists, and multidisciplinary healthcare teams to assist with activities of daily living (ADLs) and rehabilitation goals
• EHR Documentation: Maintain accurate, timely resident records within the Electronic Health Record (EHR) system in compliance with facility and safety standards

Qualifications & Requirements

Required

• Education: Graduate of an accredited Practical Nursing Program
• Licensure: Current, unencumbered LPN license in the State of Iowa (or active Compact/NLC state license)
• Certifications: Active Basic Life Support (BLS) / CPR certification
• Experience: Minimum 1+ years of long-term care or skilled nursing facility (SNF) experience
• Skills: Strong verbal and written communication skills with healthcare teams and families; proficiency in electronic health record (EHR) documentation

Preferred

• Prior travel nursing experience
• Hands-on experience with specialized wound care or palliative care principles
