Job summary

Healthforce is seeking a Licensed Practical Nurse (LPN) for a six‑week travel assignment in Gallup, New Mexico. The position runs for six weeks starting November 2, 2026, guarantees 36 hours per week, with 12‑hour day shifts from 07:00 to 19:30.

You’ll provide direct care in a skilled nursing facility, manage meds, monitor residents, and document in the EHR. Active LPN license and LTC experience preferred; local candidates are preferred; travel support and recruiter assistance provided.

Qualifications

• Active LPN license in good standing is required.
• Prior long-term care or skilled nursing experience preferred.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing facility, including routine assessments and daily monitoring.
• Administer prescribed medications and treatments, document responses, and report changes to the supervising RN.
• Monitor vital signs and intervene per facility protocol; ensure safety and comfort of residents.
• Assist residents with activities of daily living and document care accurately in the EHR.
