Job summary

St. Croix Hospice in Baxter, MN is seeking a compassionate Hospice Aide (CNA) to join our end-of-life care team. Under RN supervision, you will provide direct personal care and support to patients and families in home and community settings.

You will follow each patient’s Plan of Care, document care activities, assist with mobility, meals, and daily living tasks, and coordinate with the RN Case Manager to maintain comfort and dignity.

Qualifications

• Provide direct personal care including bathing, dressing, oral hygiene, and linen changes.
• Assist patients with mobility, transfers, and prescribed exercises.
• Support meals, feeding, and light household tasks as assigned.
• Monitor changes and report to the RN Case Manager.
• Maintain documentation and HIPAA compliance.
