Qualifications

• Minimum 2 years of Physical Therapy experience
• OASIS experience required for Start of Care assessments
• Proficiency with HCHB charting system
• BLS certification required
• Active driver's license and reliable transportation
• Strong documentation and communication skills

Responsibilities

• Provide home health PT services and develop home exercise programs
• Educate patients and families on programs, safety, and home environment adaptations
• Treat orthopedic and neurologic conditions within geriatric population
