Job summary

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent experience and proficiency with HCHB and OASIS documentation.

Responsibilities include serving a geriatric population with a caseload of 5-7 patients daily, Monday–Friday with optional weekends, and a 60-mile territory radius. Mileage is reimbursed; iPad provided; on-call every 6th weekend.

Qualifications

• Minimum 1 year of recent PT experience in home health.
• BLS certification required.
• OASIS experience required for Start of Care documentation.
• Proficiency in HCHB required.
• Active driver's license and personal transportation required.
• License required if applicable.

Responsibilities

• Home health setting serving a geriatric patient population.
• Caseload of approximately 5-7 patients per day.
• Schedule is Monday through Friday, days, with optional weekends.
• On-call required every 6th weekend.
• Territory covers up to a 60-mile radius from the hub office.
• EMR used is HCHB (Homecare Homebase).
• Responsibilities include timely documentation and communication of patient care.
• Traveler must be flexible with assigned territory.
• Scrubs required, any color, no jeans permitted.
• iPad provided; personal cell phone not required for work use.
• Mileage reimbursed at the IRS standard rate.
• Monday orientation with billable training modules completed during this time.
• First-time travelers will not be considered.
• Local candidates are not accepted.
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler.
• Travel pairs accepted, but each traveler is assigned to their own territory and must have their own transportation.
• Requested time off must be submitted at the time of application.
