Gundersen Health System in Wabasha, MN is hiring a Certified Nursing Assistant for home health travel. 0.8 FTE (32 hours/week) with base pay $19.00/hour plus CNA experience incentives, and mileage reimbursement.

You will visit patient homes within a 30-mile radius, assist with basic care, and document care plans while collaborating with the health care team. Must be Minnesota CNA and hold a valid driver's license and BLS certification.

Qualifications

• Must be a registered CNA in Minnesota.
• Valid driver’s license.
• Current Infant-Adult BLS for Health Care Providers from American Heart Association or American Red Cross.

Responsabilités

• Travel within a 30-mile radius to patient homes to assist with basic care for residents in the areas of nutrition, elimination, comfort, mobility and safety.
• Works collaboratively with the health care team members to effectively communicate and document accurate information.
