Qualifications

Responsibilities

• Assist with ADLs including meals, ambulation, turning and positioning.
• Provide physical care (baths, grooming, oral care, back care, elimination).
• Assist with admissions, discharges and transfers.
• Measure and record vital signs and intake/outtake.
• Perform perineal and incontinence care as needed.
• Observe changes and report to team.
• Maintain a clean, safe patient environment.
• Complete daily documentation accurately.

• Measure and record vital signs, weight, intake, and output information
• Perform routine perineal area and incontinence care
• Observe and report any changes in patient's general condition
• Maintain a neat and safe patient environment
• Complete documentation daily and accurately on required forms
• Flexibility in accepting different assignments
