Job summary

Codac Health Recovery & Wellness in Tucson, AZ is seeking an Outreach Engagement Specialist for the Trauma Recovery Center. Qualifications

• Masters or Bachelors degree required in healthcare or related field.
• Experience varies by degree; healthcare and non-healthcare paths acceptable.
• Related experience preferred for non-healthcare degrees.
• Certification may substitute for experience in some cases.
• Strong communication and outreach capabilities required.

Responsibilities

• Perform assertive community outreach with hospitals, EMS, and community partners.
• Build relationships with local groups, hospitals, police, and courts.
• Coordinate intake and enrollment with CODAC staff.
• Assist members with eligibility applications for entitlements.
• Provide outreach and support in homes, offices, and community settings.
• Follow up on referrals to assess outcomes and services.
• Screen individuals for TRC eligibility and make referrals.
• Coordinate transportation and accompany members to appointments.
