Job summary

CODAC Health, Recovery & Wellness, Inc. is seeking an Outreach Engagement Specialist for the Trauma Recovery Center.

The role focuses on engaging survivors of violence and trauma, including underserved populations, through assertive community outreach at hospitals, shelters, and community partners. You will build relationships with local groups, assist with eligibility processes, and support enrollment and follow-up to ensure access to CODAC services.

Qualifications

• Requires advanced knowledge of community resources and behavioral health.
• Experience with diverse populations and trauma-informed care preferred.
• Ability to document clearly and manage data.
• Driver's license required.

Responsibilities

• Perform assertive community outreach at hospitals, shelters, and community sites.
• Build and maintain relationships with community groups, hospitals, police, courts, etc.
• Provide outreach and support in home, office, and community settings; accompany to appointments.
• Assist members with eligibility applications for financial entitlements.
• Coordinate intake and enrollment with CODAC staff until completion.

Master's degree in healthcare/related field

Bachelor's degree in healthcare/related field

Certified Medical Assistant

Certified Peer Support Specialist

Certified Family Support Partner

EMT (optional)
