Job summary

Intermountain Health is seeking a Clinical Program Coordinator (RN) for Good Samaritan Hospital in Lafayette, CO. You will guide patients through the care continuum, coordinate with physicians and nurses, and provide education during hospital stay while serving as a resource after discharge.

The role is full-time, 40 hours/week, with Monday–Friday shifts; on-call may be required. A BSN, current RN license, BLS-HCP, and 3+ years of RN experience are required; trauma experience is preferred.

Qualifications

• Bachelor's Degree in Nursing required.
• Current RN license in the state of practice.
• BLS-HCP certification on hire or within 60 days.
• Three years of RN experience required.

Responsibilities

• Coordinate and discuss the program with physicians and nurses and ensure care meets evidence-based guidelines.
• Educate patients and families during hospitalization and serve as a contact after discharge.
• Participate in patient care rounds and support performance improvement activities.
• Develop and implement evidence-based policies and standards for the program.
• Maintain program data and act as subject matter expert; support quality assurance.
• Promote the mission and values of Intermountain Health; may float to other departments and be on-call.
