Job summary

HeartShare is seeking a dedicated Success Coach to join the HSVS American Dream Coaching Program, providing personalized educational, employment, and housing support to foster youths and graduates. Based in Queens and Staten Island, you will build trust, set goals, and use a trauma-informed approach to help youths progress toward independence and post-secondary plans.

You will partner with housing and education teams, facilitate workshops, and maintain accurate data while advocating for youth

Qualifications

• Bachelor's degree required.
• 1–2 years of experience in education and foster care systems.
• Experience supporting IEPs and 504 plans.
• Excellent verbal and written communication skills.
• Experience with youth and families experiencing serious emotional disturbance.
• Flexibility to work evenings and travel across boroughs.
• Proficient with Microsoft Office and databases (e.g., Microsoft Access).

Responsibilities

• Build relationships with youth in care or formerly in care.
• Collaborate with case planning, Permanency Units, Education, and Youth Development programs.
• Support a roster of youth as directed by supervisor and director.
• Conduct educational assessments and draft plans for youth and teams.
• Update goal worksheets and data on Care4 platform weekly.
• Complete progress notes on Care4 and Connections for all youth.
• Plan and facilitate Preparing Youth for Adulthood workshops and activities.
• Provide crisis intervention and trauma-informed support.
