Job summary

Heartshare is seeking a Social Worker to support youth in two group-home settings operating under ACS and OCFS. You will provide counseling, crisis intervention, and trauma-informed care while coordinating with residential leadership and interdisciplinary teams across Queens and Staten Island.

The role emphasizes evidence-based practice (CBT/DBT/MI), intake and service planning, and collaboration with external providers to ensure seamless, holistic care for twelve youths per home.

Qualifications

• Master’s degree in Social Work (MSW) required.
• 2–4 years of experience working with youth in residential programs.
• Experience with CBT/DBT/MI or similar evidence-based modalities.
• Ability to provide psychoeducation to staff and collaborate within teams.
• Work across two program sites with a dynamic caseload.

Responsibilities

• Provide individual counseling, crisis intervention, and ongoing youth support.
• Deliver group therapy and social–emotional skill building activities.
• Complete intake, assessments, and documentation per ACS/OCFS standards.
• Coordinate with families, schools, and external providers to ensure continuity of care.
• Collaborate with ITS team to integrate trauma-informed practices.
