Job summary

Heartshare is seeking compassionate Coaches to support homeless or at‑risk youth in NYC through the Evolve Youth Time Limited Rental Assistance Program. You will guide youths toward rapid housing with private landlords, using a strengths‑based, collaborative approach and CTI methods to build independent living skills.

You will conduct intakes, needs assessments, and bi‑weekly coaching, while cultivating partnerships with landlords and services.

Qualifications

  • Bachelor's degree from four year college or equivalent required.
  • NYC Driver’s License required.
  • Knowledge and understanding of homeless population.
  • Experience conducting groups and developing youth programming.
  • Comfort traveling throughout NYC.
  • Work flexible hours.

Responsibilities

  • Complete intake with youth and referrals to agencies.
  • Complete needs assessment including mental health and social history.
  • Assist with housing applications, employment, budgeting and schooling.
  • Develop and participate in skill-building groups.
  • Provide bi-weekly individual coaching and referrals to outside services.
  • Maintain active caseload and document CTI goals and stages.
  • Engage landlords and community partners; report results to team.
  • Upload plans and CTI-related documentation.

Compassionate, collaborative, and engaging Coaches are needed at The Evolve Youth Time Limited Rental Assistance Program. The position is designed to work with individuals experiencing homelessness to find rapid housing with private landlords. Coaches provide guidance, coaching, and support to vulnerable young adults who are at risk of homelessness, to help them achieve permanent housing with the support of time limited rental assistance. Coaches will adopt a nonjudgmental, strengths-based, and collaborative approach to working with youths, ensuring that their wishes, interests, and preferences inform the development of their service support plan goals. Coaches will collaborate with their team to ensure that youth are well equipped to navigate independent living. This will include crisis intervention entitlements procurement, health care, mental health, independent living skills, job/career assistance, financial literacy, recreational opportunities, education journey and other services as needed. Coaches will use the principles of positive youth development to ensure that youths’ goals and services are focused on building independent living skills, resiliency and social connection which will ultimately ensure independence. Coaches will empower youth to engage in all aspects of the process and teach them how to advocate for themselves and prevent or troubleshoot obstacles. Coaches will develop a robust network of service providers to ensure that youths are able to access the services that they desire. Coaches will be trained in and implement evidence-based models, including:

  • Motivational Interviewing
  • Recovery Oriented and Trauma Informed case management
  • Critical Time Intervention
  • Motivational Interviewing
  • Intensive Trauma Services
  • Recovery Oriented and Trauma Informed case management
  • Critical Time Intervention

Coaches will undergo a comprehensive training program to ensure that they are trained in evidence-based practices, as well as in conflict resolution, crisis intervention, crisis de-escalation, harm reduction, person-centered planning, assessment, housing first approaches, mandatory reporting and recognizing signs of abuse and neglect, co-occurring disorders, and recovery-oriented practices.

Essential Functions And Primary Responsibilities Of Position
  • In addition to responsibilities below, performs other job-related duties as assigned, consistent with the scope, responsibilities, and level of the position, and in compliance with applicable laws, regulations, and organizational policies.
  • Participates in in‑service trainings and on‑site workshops as required for professional growth and development. Attends external trainings and workshops at direction of management
  • Complete Intake process with youth according to established guidelines and make appropriate contact with and /or referrals to other agencies
  • Complete needs assessment including but not limited to mental health and social history.
  • Obtain legal documents (birth certificates, NYS Identification, social security), assist with housing applications, assist with employment, budgeting, and enroll and in school where applicable.
  • Facilitate, develop, and participate in skill building groups etc…
  • Provide bi -weekly individual coaching to each youth. Making referrals to outside services when needed such as substance abuse, mental health, etc..
  • Keep updated chart records for all current and past residents and complete all mandatory paperwork required by contracts and funding sources.
  • Work alongside all other staff to develop and evaluate programming. Conduct outreach presentations as needed and assigned.
  • Conduct monthly home visits in accordance with CTI Model.
  • Attend Regular staff meetings and trainings to support the needs of the youth.
  • Upload support plans, and relevant documentation that reflects CTI goals and stages.
  • Maintain an active caseload of 15 + tenants.
  • Manage tenant referrals and follow up with phone calls.
  • Cultivate and foster working partnerships with community landlords.
  • Prepare and track documentation including tenant services via Eccovia Data System.
