Job summary

Alliance for Community Empowerment's Reentry Action for Youth (RAY) program in Los Angeles seeks mentors to support youths facing barriers. You will use trauma-informed approaches to coordinate care, connect to services, and guide participants through complex systems.

You will maintain regular contact, manage a caseload, document interactions, and collaborate with attorneys and probation officers to help youths complete services and re-enter their communities.

Qualifications

• High School Diploma and community intervention experience.
• Experience with individuals facing system involvement, mental illness, substance use, or homelessness is a plus.
• Spanish language skills preferred.

Responsibilities

• Mentor, motivate, and empower youth participants to become positive community members.
• Maintain consistent contact with youth participants, including in-person meetings.
• Manage a caseload of approximately 10-12 youth, with individualized support.
• Conduct intake and exit assessments and develop care plans.
• Document all interactions and maintain case notes and care plans.
• Provide linkages and referrals through individualized care planning.
• Develop relationships with service providers for referral pathways.
