Job summary

GRAND Mental Health is seeking a Behavioral Health Skills Coach to assist youth and families in overcoming barriers affecting emotional, behavioral, and educational outcomes. The role emphasizes person-centered, trauma-informed care aligned with treatment plans, with certification maintenance and strong documentation practices.

This FT non-exempt position requires collaboration with schools and community partners, documentation via Eleos, and adherence to confidentiality and ethical standards.

Qualifications

• Case Management II Certification OR equivalent paths.
• Must obtain and maintain required certifications and training for service delivery.

Responsibilities

• Provide behavioral health coaching, rehabilitative services, wellness education, and case management to youth and families.
• Assist youth in achieving success at school, home, and community.
• Provide one-on-one skills training focused on behavioral regulation, communication, daily living, emotional wellness, and coping strategies.
• Deliver curriculum-based wellness and rehabilitative interventions as per treatment plans.
• Provide outreach, engagement, and recovery support services to youth and families.
• Support implementation and reinforcement of treatment goals and skills learned through treatment.
