Job summary

Qualifications

• Bachelor's degree required in Social Work, Psychology, Human Services, Child Development, Sociology, or a related behavioral health field.
• Minimum one year of case management, advocacy, or direct service experience with at-risk youth.
• Bilingual English/Spanish preferred; strong writing and communication skills.

Responsibilities

• Advocacy and youth engagement, building trusting relationships across community settings and services.
• Crisis response, safety planning, de-escalation, and coordinated support for high-risk youth.
• Case management and coordination with DCFS, DMH, schools, and partner agencies.
• Documentation of progress notes, plans, surveys, and required program records.
• Participation in supervision, team meetings, MDTs, and multidisciplinary planning.
• Maintain professional conduct, confidentiality, and trauma-informed care standards.
