You will work with a care team to implement treatment plans, complete progress notes, and support residents toward their goals.

Responsibilities include coordinating referrals, accessing treatment, and ensuring trauma-informed practices.

Qualifications

• Bachelor's degree in social work or related field.
• Experience in residential behavioral health settings preferred.
• Strong trauma-informed approach and confidentiality.
• Ability to document accurately in EMR and care plans.

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents; coordinate services and document plans.
• Coordinate referrals and intake, ensuring timely progress on individualized plans.
• Collaborate with the care team to arrange transportation and appointments for residents.
• Maintain trauma-informed care across all interactions and documentation.
• Support on-site operations, including shifts, shifts handoffs, and team communication.
• Ensure proper medication access and safe administration following training.
