Qualifications

• Bachelor’s degree required or 2+ years of relevant experience for adults with severe mental illness
• Pass/criminal background checks and health/safety requirements
• Ability to navigate electronic medical records and coordinate with care teams

Responsibilities

• Serve as Primary Case Manager for assigned caseload and maintain up-to-date service and crisis plans
• Coordinate referrals and timely intake; develop individualized plans
• Collaborate with care team to ensure access to treatment, support, and appointments
• Arrange transportation and conduct home/community outreach for stability
• Maintain confidentiality and adhere to trauma-informed care principles
• Manage medications coordination and ensure documentation to support billing
• Contribute to discharge planning and care team communications
• Provide after-hours crisis intervention when required
