Job summary

Community House Mental Health Agency in Seattle, WA is seeking a dedicated Diversion Residential Case Manager. You will serve as a secondary case manager and on-site contact for residents, coordinating plans, referrals, and transportation to services.

You will uphold trauma-informed care principles, ensure proper medication processes, and document interactions in the EMR. This role offers a supportive team, competitive hourly pay, and comprehensive benefits.

Qualifications

• Must apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days.
• Criminal background check required prior to hire.
• First Aid/CPR certification due within 30 days of hire.
• TB test completed within first 2 weeks.
• Valid license with clean driving record for agency vehicles.

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents, advocating for and coordinating needs and services.
• Coordinate referrals, ensure timely intake, and review individualized plans.
• Collaborate with care team to ensure access to treatment, support, and appointments, including transportation.
• Ensure trauma-informed care principles in interactions (safety, choice, trust, collaboration, empowerment).
• Manage medication access following seven rights and document accordingly.
• Participate in shift change meetings and complete daily EMR documentation.
• Practice financial stewardship of resources and monitor petty cash usage.
• Meet KPIs and contribute to discharge planning and care summaries.
• Assist with on-site cleaning, training residents, and maintenance of spaces.
