Job summary

Community House Mental Health Agency is seeking a Diversion Case Manager in Seattle to provide outpatient behavioral health services and care coordination within their diversion program. You will advocate for clients, coordinate services and documentation, and work with the care team to ensure timely treatment and discharge planning.

Responsibilities include direct case management, referrals, transportation coordination, crisis intervention when needed, and maintaining confidential client

Qualifications

• Bachelor’s degree or 2+ years relevant experience serving adults with severe and persistent mental illness or in diversion services.
• Credentialing as Certified Peer Counselor acceptable/preferred for FHARPS program.
• Criminal background check and First Aid/CPR certification required.

Responsibilities

• Serve as Primary Case Manager for assigned caseload, maintaining service and crisis plans on time.
• Coordinate referrals and intake, develop individualized plans.
• Collaborate with care team to ensure access to treatment, support, and appointments.
• Provide outreach in clients' homes or community to assess living stability and provide interventions.
• Follow trauma-informed care principles in all interactions.
• Coordinate medication access following seven rights.
• Complete daily EMR documentation for all interactions and activities.
• Monitor resources and ensure proper use of funds and assets.
• Meet program KPIs and participate in discharge planning and information sharing.
• Provide after-hours crisis intervention as needed.
