Job summary

LifeMoves is seeking a Bayside HOT Case Manager to support neighbors moving toward safe, stable housing. You will provide trauma-informed case management, life skills coaching, and resource connections in partnership with clients to set goals and overcome challenges.

You will build trust through empathetic, client-centered support, coordinate housing and benefits access, and collaborate with community partners to strengthen services while documenting progress in HMIS and internal systems.

Responsibilities

• Provide client-centered case management and life skills support.
• Connect clients to housing, healthcare, benefits, and community resources.
• Engage with community partners to strengthen services and advocate for clients.
• Maintain accurate client records and program data in HMIS and internal systems.
• Provide crisis intervention and safety planning as needed.
• Participate in trainings, case conferences, and supervise to improve practice.
• Attend occasional evenings and weekends as schedule requires.
