Qualifications

• Bachelor’s degree in social science or related field; 2+ years may substitute.
• Certified Peer Counselor credential preferred and encouraged.

Responsibilities

• Serve as Primary Case Manager for assigned caseload and coordinate client needs and services.
• Ensure Service Plans and documentation are completed on time and up-to-date.
• Collaborate with internal/external teams to coordinate client referrals and intake.
• Assist with treatment, case management, and transportation to appointments as needed.
• Maintain confidentiality and trauma-informed care principles in all interactions.
