Job summary

ICL Human Services is seeking a dedicated Case Manager to support adults and transition age youth with mental health needs in New York. You will develop and monitor individualized treatment plans, provide one-on-one and group counseling, and coordinate with medical and social services to promote recovery and independent living.

The role emphasizes trauma-informed, client-centered care, cultural sensitivity, and thorough documentation.

Qualifications

  • Bachelor’s degree in a human services field or equivalent experience.
  • Two years of full‑time experience providing direct services to individuals with mental disabilities or related human services.
  • Knowledge of mental illness, substance use disorders, treatment, rehabilitation, and community support programs.
  • Ability to work with diverse populations with sensitivity and respect.

Responsibilities

  • Develop, evaluate and modify consumer treatment plans and quarterly reviews.
  • Provide supportive counseling in 1:1 or group formats.
  • Maintain accurate case records and document progress toward goals.
  • Assist with discharge planning and linkages to community resources.
  • Ensure medication adherence and accompany consumers to appointments as needed.
  • Perform crisis assessment and intervention when required.
  • Participate in team meetings, trainings, and protocol reviews.
  • Report incidents and maintain safety and confidentiality in all duties.
